Registration and Payment
The CPAT Testing Fee is $200.
Please review the Candidate Physical Agility Test (CPAT) FAQ Section to answer any questions you may have regarding the CPAT testing process and registration. The only person allowed in the Fire Station and/or on the CPAT course is the testing participant. We are not able to accommodate additional people other than testing participants and on-duty staff.
To register for a CPAT testing process, correctly complete and submit the online registration form located on the Orientation & Time Trial page. Then mail your application and the $200 payment (checks payable to West Allis Fire Department) to: West Allis Fire Department; 7332 W. National Ave., West Allis, WI 53214.
If you are having trouble emailing the form, please review #9 in the FAQ section for direction.
Correctly completed applications can also be dropped off at the above address, faxed to 414-302-8927 or emailed to firstname.lastname@example.org.
Once your application has been received, an email will be sent to you confirming your selected dates as well as additional information regarding the testing process. If you do not receive a confirmation email, that means your application has not been received and you are not registered for a CPAT testing process.
Applicants must be at least 18 years of age, and in good physical health. Please see the below CPAT Orientation Guide and CPAT Preparation Guide to help prepare you for this testing process.
CPAT Payment Requirements
The CPAT testing fee is $200. This fee is payable by personal check, cashier’s check or cash. Checks should be made payable to the West Allis Fire Department and mailed with your completed CPAT application. Payments are non-refundable and cannot be transferred or carried over. Payment is due within ten days of registering for a CPAT testing process or your name will be removed from the participant list. Payments for late registration (less than 10 days of a time trial or test) will be accepted in the form of cash, exact amount only at your first registered time trial or test.
Waiver of Liability
All candidates must sign a waiver of liability prior to participating in an orientation session, time trial or test. Candidates may not participate in an orientation, time trial or test without a signed, valid waiver. These waiver forms will be available on the day of the orientation, time trial or test.
