CPAT Payment Requirements

The CPAT fee is $150. This fee is payable by personal check, cashier's check or cash. Checks should be made payable to the West Allis Fire Department and mailed with your completed CPAT application. Payments are non-refundable and cannot be transferred or carried over. Payment is due within ten days of registering for a CPAT testing process or your name will be removed from the participant list. Payments for late registration (less than 10 days of a time trial or test) will be accepted in the form of cash only at the first time trial/test you are registered for.

Waiver of Liability

All candidates must sign a waiver of liability prior to participating in an orientation session, time trial or test. Candidates may not participate in an orientation, time trial or test without a signed, valid waiver. These waiver forms will be available on the day of the orientation, time trial or test.
