2017 Open Enrollment Information
Please choose one of the following for complete 2017 Open Enrollment information:

It's time once again for the City's annual benefits open enrollment period, which begins January 30th and runs through 5 p.m., February 8th, 2017.  You are encouraged to review the benefits available and to enroll early.

Please remember, you are required to make an election on the enrollment forms for Health and Dental Insurance even if you are choosing not to participate.

2017 Information
For 2017, the City will remain with the same health plan administrator: Anthem Blue Cross and Blue Shield.  You continue to have the choice of two Health Plan Options:

Anthem PPO Plan - Blue Priority Network

Plan Summary | Active Employees
Plan Summary | Retirees (Retired on or after 3/1/13)
Plan Summary | Retirees (Retired Prior to 3/1/13)


Anthem HDHP Plan with HSA - Blue Preferred Network

Plan Summary | Active Employees
Plan Summary | Retirees

The 2017 Open Enrollment Guide is a great resource to help you determine the right coverage to meet your needs for the coming coverage year.  Please take the time to carefully review your options and make choices that will help you and your family stay healthy.  You are invited to attend an open enrollment meeting or view a videotaped session.

Tri-City HSA
Individuals who currently participate in an HSA do not need to open a new account.  So while the HSA enrollment form, Tri-City Application and Customer Due Diligence Questionnaire need to be completed by new members, only the City’s HSA enrollment form needs to be completed for current participants if they choose to continue with such.
